Hotel Metro Rooftop Bar [ Bathroom Rating: A; Bar Rating: B+ ]
One from the summer...
The Metro Hotel is at 45 W 35th St (btw 5th and 6th Ave). There are two unisex bathrooms. Both are a good size with lockable doors. Functional sink and hot water, lots of hand towels and the bathroom was clean.
Cool roof bar, great view of the Empire State Building, recommended. The roof bar is only open Monday through Friday during the summer from 6pm to 10pm.

WCOU [ Bathroom Rating: A; Bar Rating: A ]
WCOU is at 115 1st Ave (cross: 7th St). The mens bathroom is small with a toilet and a urinal but room only for one. Lockable door. Functional sink with hot water, hand towels and the bathroom was sufficiently clean.
Cool bar, good jukebox, recommended.

Holiday Cocktail Lounge [ Bathroom Rating: A; Bar Rating: A ]
The Holiday Cocktail Lounge is at 75 Saint Marks Pl (btw 1st and 2nd Ave). The mens bathroom is small with a toilet and a urinal but really only enough room for one. The door was lockable. Working sink with hot water, plenty of hand towels and the bathroom was sufficiently clean.
This is a cool bar, perhaps a fav of mine now, plus $3 beers can be tough to find in NYC.

Jake's Saloon [ Bathroom Rating: C+; Bar Rating: C+ ]
Jake's Saloon is a restaurant bar on 206 W 23rd St (between 7th and 8th Ave). It has bathrooms in the back for men and women. The mens bathroom is large with a stall and a couple urinals, working sinks with hot water and a hand dryer. The bathroom was pretty dirty, someone pissed on the floor and there was puke in the corner. Minus these unfortuneate additions it has the potential to get and "A" rating.

D J Reynolds [ Bathroom Rating: A; Bar Rating: C+ ]
DJ Reynolds is a restaurant bar on 351 W 57TH St (between 8th and 9th Ave). It has two bathrooms in the back; one for men one for women. The mens bathroom is large, clean, working sinks, hot water and paper. It has a stall and a urinal.

How I rate bar bathrooms and bars
I'll give bar bathrooms a rating "A" through "F". It's totally subjective of course but I'm looking for things like cleanlines, space, working toilets, sinks that work with hot and cold water and have paper/hand dryer/soap. If a bathroom has these things, they'll generally get an "A". I'll note the rating in the title of the entry.
I'll also give my rating for the bar itself using the "A" through "F" approach. Again totally subjective but I'm not a fan of over-priced drinks, super loud bars, waiting in line to get in or waiting forever to get a drink. Flight 151 [ Bathroom Rating: A; Bar Rating B- ]
Flight 151 is a sports bar in Chelsea at 151 8TH Ave (between 17th and 18th). It has two bathrooms in the back, single toilet in each. Flight 151 bathrooms are good, large space, clean, working sinks, hot water and paper.
I went to Flight 151 to catch a football game with a friend that leaves in the area as it has plenty of TV's. It's a pretty standard sports bar but I have not been back.

Beauty Bar [ Bathroom Rating: A; Bar Rating: B ]
The "Beauty Bar", 231 E 14th St (btw 2nd and 3rd Ave), has two small bathrooms, single toilet in each. The bathrooms were good, enough space, fairly clean with sinks that worked, had hot water and paper (plenty for your hands and privates).
Beauty Bar is a fine bar but it's unlikely I will frequent it. It's a little too hip, too young, and it's loud. I will say the dance floor in back is cool, the DJ was good and they've kept the beauty salon chairs, chrome hair dryers and other salon hardware from the original Thomas Beauty Salon which does add nicely to the ambiance giving the place a neat edge.
Low light so bad photo, I guess what can you expect from a camera phone.
