Manhattan Bar Bathrooms
The Olde Castle Pub [ Bathroom Rating: B; Bar Rating: B+ ]

The Oldcastle Pub is at 160 W 54th St closer to 7th Avenue. The mens room is in the back and has a stall and a urinal. I wouldn't call the bathroom clean and the sink offered no hot water and there were no hand towels or dryer. The bathroom was really bright. I don't care about that too much but it did feel like I was taking a leak with a spotlight on my back. I'd recommend they install lower wattage light bulbs.
Olde Castle is a good Irish bar. Nice place to get something to eat, enjoy a good beer, and chat comfartably. Food was pretty good to boot.

Black Door [ Bathroom Rating: A; Bar Rating: B+ ]

The Black Door is at 127 W 26th St (btw 6th and 7th Avenue). There are three unisex single stall bathrooms near the back. Nice bathrooms, plenty of space, sinks with hot water and towels.
I like the Black Door. It can be loud but a good place with good space, recommended. Good bartenders too, after a couple rounds they've given me drinks on the house; and on more than a couple occasions.

XR Bar [ Bathroom Rating: B?; Bar Rating: C- ]

XR Bar is at 128 W Houston St, at the corner of Sullivan Street. I forget the details of this bathroom bar but I took a picture; I sort of recall it as OK.
This bar was loud and busy, not a fan, unlikely I'd go back.

Tillman's Bar and Lounge [ Bathroom Rating: B+; Bar Rating: B+ ]

Tillman's Bar & Lounge is at 165 West 26th St (btw 6th and 7th Avenue). The mens room is in the lower level. It has two side by side urinals and a stall. The stall seemed unavailable as the door was locked but there seemed to be no one in there. Bathroom was clean with a functional sink and hot water with sufficient hand towels. Not a fan of side by side urinals as I don't want to be rubbing elbows with someone when I'm taking a leak.
Tillman's was pretty cool with a Harlem and jazz scene, but I'm not sure what to think of it, I felt it wants to be cooler than it is.

Scratcher [ Bathroom Rating: A; Bar Rating: A ]

Scratcher is at 209 East 5th St (cross Bowery). The mens room is a single and it was clean with functional sink (a shared sink) with towels with warm water at least.
Scratcher is a good Irish bar, great place for a relaxing drink. Not loud, can chat easily without the need to shout.

Guero [ Bathroom Rating: B; Bar Rating: A- ]

Guero is located at 9 Ave A (btw 1st and 2nd Street). Guero is in the basement, bathroom is upstairs in the main bar (which has a different name that I forget). The bathroom is a single. It was pretty clean with functional sink but no towels and no hot water. Had to use TP to dry my hands.
Guero is a fun place with a group of folks, DJ with dancing. When you're tired of that there's a pool table upstairs, with women dancing on the bar (not sure if that happens every night but was going on when I was there).

The Dive Bar [ Bathroom Rating: B+; Bar Rating: A- ]

The Dive Bar is located at 732 Amsterdam Ave (at 96th Street). The bathroom has a stall and a urinal. Though quite cramped it was reasonably clean with functional sink and towels. No hot water.
I like The Dive Bar and recommend it. Good spot for a drink and to relax. Been here a couple times and generally it's not crowded, good bar food too.

Sasha's Bar and Lounge [ Bathroom Rating: A-; Bar Rating: B+ ]

Sasha's Bar and Lounge is at 255 W 55th Street just off Eighth Avenue. The bathroom has a stall and two urinals. It was reasonably clean with functional sink and towels. No hot water though.
I liked this place. Cozy, dim lighting with Russian propaganda art. Good DJ with space to dance.

Sidewalk Cafe [ Bathroom Rating: B+; Bar Rating: B+ ]

Sidewalk Cafe is at 94 Avenue A at the corner of Avenue A and 6th Street. I used the mens batroom on the lower floor, sort of a nuisance, not sure if there was one on the main floor. Bathroom had two side by side urinals and a lockable stall. The bathroom was fairly clean, stall was a bit messy. Sink had hot water and plenty of towles.
Sidewalk Cafe is a cool place, there's an intimate stage in the back for live music, good atmosphere.

This is the "Manhattan Bar Bathrooms" Blog. Here I'll discuss NYC bars I've been to and comment on the condition of their bathrooms, even providing a picture of the bathroom. I want to track, for my benefit and yours, which bar bathrooms in the city are decent and which are shitholes.
